In this role you will engage with people that have substance use issues. The clients might have experienced a range of complex health needs. You will assess and provide support on social needs and welfare rights.
You will work flexibly, seeing clients at the static sites and visiting clients in their own homes or on outreach in all areas of Leeds and provide them with a range of support to help them to reduce the harm to themselves and others.
You will be responsible for managing the support for several clients as well as providing cover for staff. The role involves lone work, responding to the individual needs of the people that you support, as well as working as part of our wider team.
