As a Recovery Coordinator for Forward Leeds, you will be an integral member of the dynamic multi-agency team developing comprehensive and tailored individual interventions that will include delivery of holistic support to adult service users and to coordinate their drug treatment, including assessment, recovery coordination, PSI delivery and multi-agency working.
Working within the York Street Health Practice primary care enhanced service. Engaging and supporting those who are homeless and present with complex needs including substance misuse, physical and mental health needs, to build recovery capital and progress towards stability with recovery focused goals.
