This is an interesting and rewarding role. The post holder will provide holistic support to adults to support their drug/alcohol treatment.
You will have the flexibility to complete greater levels of outreach and support people in reducing barriers to engagement.
As an Enhanced Care Practitioner, you will support high-need people who are unable to progress in their timeframe due to their multiple needs. Multiple needs include but are not limited to, chronic physical health, comorbidities, financial issues, memory/cognitive impairment, age, housing, limited social support, and other multi-agency support needs.
