This role will be based at our busy city centre hub. Support will include preparation for those wishing to receive treatment via an in-patient detox unit such as St Anne’s Alcohol Services, community detox or out-of-area pathways.
You will also use other service provisions within Forward Leeds and the wider community. You will ensure that the service user and their family have access to appropriate care coordinated in line with a person-centred approach.
You will work closely with the Team Manager and be responsible for your caseload of service users. You will organise face-to-face appointments and deliver therapeutic sessions including safe alcohol reduction plans, risk assessments and harm reduction/relapse prevention work. You will also link in with GP’s, Healthcare Assistants, hospital teams, and rehab facilities to work holistically in supporting services users achieve their planned treatment goals.
