We have an excellent opportunity for an alcohol and drug practitioner, who is able to work on their own initiative from GP practices across the city, to help deliver a partnership model of working between the CCG’s and Forward Leeds.
The successful candidate will link in with a number of different surgeries from where they will motivate patients to make positive changes to their drinking or substance misuse. They will have a base in one of our hubs when not in GP practices, and help contribute to the excellent recovery focussed outcomes that Forward Leeds delivers. They will need to be able to travel across the city and thus require their own transport.
