We are seeking to recruit a Harm Reduction Social Support Practitioner, to engage with people that have substance use issues. The service-users could be experiencing a range of complex health and social needs issues. You will assess, act on immediate risk and collaboratively create a care plan.
You will cover the needle exchanges both from Forward Leeds’s Hubs and on Outreach. Assertive Outreach will be provided, for service-users that are struggling to engage or at a time of increased vulnerability. This will be covered with colleagues. You will provide the service-user with a range of support to help them to reduce the harm to themselves and or others. You will hold a dynamic caseload of service-users that you are supporting with benefits, welfare rights of housing issues. You will work closely with all aspects of the Forward Leeds Alcohol and drugs treatment consortium scheme and mental health services.
You must be highly motivated, be willing and enjoy working as part of a team. You will be responsible for managing the support for clients as well as providing cover for staff. You will be able to demonstrate the ability to lone work and respond to the individual needs of the people that you support.
