Forward Leeds Young People’s Team are looking to recruit a passionate, creative Lead Practitioner for its team working with 18 – 24-year-olds. You will work closely with the Service Manager and the Lead Practitioner for under 18’s to ensure people who use the service receive positive, life-changing interventions.
Some of the key duties will include:
- Establishing city-wide multi-agency links to increase referrals into the service.
- Manage a caseload of service users, completing and reviewing high quality and comprehensive assessments, risk assessments and support plans, including liaising with family members, significant others and professionals / agencies as appropriate.
- Implement, develop and assist in the management of services delivered by the team.
- Manage, mentor, coach and ensure the development of staff reporting to you.
