The Detox and Rehab team are expanding! Working within a team located across three sites in Leeds you will support and provide a range of interventions to people affected by alcohol and substance use, though the role is primarily alcohol led.
Support will include preparation for those wishing to receive treatment via an In Patients Detox Unit such as St Anne’s Alcohol Services, Community Detox or Out of Area pathways. You will also make use of other service provisions within Forward Leeds and the wider community. Our aim is to ensure that the service user and their family have access to appropriate care which is coordinated in line with a person-centred approach.
You will work closely with the Team Manager and will be responsible for your own case load of service users. You will organise face to face appointments and deliver therapeutic sessions including safe alcohol reduction plans, risk assessments and harm reduction/relapse prevention work. You will also link in with GP’s, Healthcare Assistants, Hospital Teams, and Rehab Facilities to work holistically in supporting services users to achieve their planned treatment goals.
