The team is well-established and has a large amount of clinical credibility within one of the country’s largest NHS trusts.
The primary function of the team is to provide a liaison role between the Leeds Teaching Hospitals Trust, Forward Leeds and users of their services. This involves communicating any updates, regarding the physical or mental health of services users, to the service user or other relevant people involved to ensure safe delivery of care. The hospital will also ensure that any treatment initiated by Forward Leeds is continued in hospital and conversely ensure that this continues smoothly on discharge.
The Hospital team work in partnership, and regularly do joint assessments, with a number of different specialities within LTHT such as the hospital mental health teams, ALPS, the pain team, social services and safeguarding.
