We have an excellent opportunity for an alcohol and drug practitioner with experience, who is able to work on their own initiative from GP practices across the city, to help deliver a partnership model of working between the CCGs and Forward Leeds. The successful candidate will link in with a number of different surgeries from where they will motivate patients to make positive changes to their drinking or substance misuse.
They will have a good knowledge of harm reduction messages, risk management, recovery planning, therapeutic interventions and be able to support clients to address their alcohol and drug (non opiate) problems. They will need to be confident in their abilities, have good communication and record-keeping skills and be self motivated, as they will often be working remotely from the rest of the team. As part of the role, they will need to build on established relationships within existing practices as well as develop the service in new GP Practices.
