Forward Leeds is part of a city-wide outreach team offering support, care, and guidance to those homeless. We have a new and exciting opportunity as part of a new initiative for an experienced Independent Prescriber to join our Street Outreach team. The successful candidate will offer rapid access to OST and support individuals to access further clinical treatment. There will also be an element of health assessment, advice, and support. You will work alongside a variety of street based complex needs and support workers as well as a psychologist, to ensure a holistic offer to these clients. This role will include some unsocial hours to ensure we reach those in most need.
